2025-2029 State Health Improvement Plan 

The Office of Public Health Practice, Planning, and Local Partnerships recently led a collaborative assessment and planning process that created CDPHE’s 2025-2029 State Health Improvement Plan. The 2025-2029 plan follows the comprehensive 2023 State Health Assessment and sets priorities for the public health system for the next five years. Over 60 subject matter experts, including both CDPHE directors and staff as well as external partners, participated in this process.

When your CHA or CHIP are completed and approved by your local board of health, you can submit them to our office using this CHA/CHIP Google Submission Form

CPHS Resource Guide

The new Colorado Core Public Health Services Resource Guide is a great tool for understanding the functions of each CPHS. It also includes helpful shortcuts to related CDPHE programs and offices.

As a local public health agency staff, you have access to the Public Health Digital Library where you can access a variety of peer reviewed journals and databases for free.

LPHA Document Repository

OPHP has created a document repository for Local Public Health Agency staff to find and share locally-developed resources related to the Core Public Health Services, including Assessment and Planning. All agencies are invited to submit documents for other LPHAs to use and learn from. We hope this shared space will give LPHAs a chance to connect and collaborate across the state.

Do you have documents to share? To ensure resources are appropriately placed and shared within the repository, all documents must be submitted through the LPHA Document Repository Google Form
